We commend David J Lewis and colleagues1 for demonstrating that ibrutinib–rituximab can reach chemotherapy-free disease control in older patients with mantle-cell lymphoma who were not eligible for transplantation. The ENRICH trial represents a pivotal advance, yet its findings invite refinement for broader clinical practice. First, the absence of minimal residual disease (MRD) monitoring reduces the opportunity to identify patients who could safely discontinue Bruton tyrosine kinase (BTK) inhibition once deep molecular remission is reached.
