Rethinking Global Sex Differences in Life Expectancy

New research shows that disparities in lifespan between men and women vary across countries and are influenced by social and environmental factors as much as biology.

While women typically outlive men worldwide, recent studies reveal that the magnitude and direction of this gap depend on how it is measured. A study published in PLOS Medicine introduces an adjusted measure of life expectancy differences that accounts for what is achievable under optimal health conditions. This approach allows for cross-country comparisons that separate biological factors from inequalities arising from preventable health and social determinants.

The study analyzed life expectancy data from 237 countries at multiple ages, from birth to 70 years. Researchers identified “frontier” countries with the highest male and female life expectancy and benchmarked all other countries against these values. The resulting adjusted sex ratios highlight where men or women are disadvantaged relative to potential rather than raw averages. Globally, men remain at a disadvantage in most regions, but in some countries, particularly in South Asia and parts of Africa, women experience shorter life expectancy at specific ages. These findings reveal complex, age- and region-specific patterns of inequality.

Measuring Life Expectancy Inequalities

Traditional analyses often report the absolute difference in average lifespan between sexes, which can obscure disparities driven by avoidable factors. By comparing countries to frontier benchmarks, the adjusted measure identifies where health outcomes deviate from achievable standards. This method provides a more nuanced understanding of life expectancy gaps and highlights populations where interventions could be most effective.

The adjusted sex ratio offers a tool for researchers and policymakers to assess global health inequalities more accurately. It underscores that sex differences in lifespan are not purely biological but are shaped by healthcare access, environmental conditions, and social determinants.
