Proliferative vitreoretinopathy and outer retinal folds in rhegmatogenous retinal detachment. Insights into retinal remodeling.

Abstract

To evaluate the relationship between proliferative vitreoretinopathy (PVR) and outer retinal folds (ORFs) in eyes with rhegmatogenous retinal detachment (RRD).
Prospective cohort study. Patients with primary RRD operated with pars plana vitrectomy were studied postoperatively by using blue fundus autofluorescence and spectral-domain optical coherence tomography. Primary outcome was the influence of PVR severity on the incidence and height of ORFs at 1 month after surgery .
The study included 171 eyes. One month after operation, ORFs were identified in 22% of the eyes with PVR A, 26.6% with PVR B, and 44.4% with PVR C. The mean height (± SD) of ORFs was 99.8 ± 18.3 µm, 138.3 ± 50.3 µm and 199.25 ± 200.0 µm in PVR A, B and C, respectively (P = 0.008). Variables associated with the presence of ORFs included PVR grade C (p = 0.004), presence of ERM (P = 0.004), and gas tamponade (P = 0.02). PVR grade C correlated with ORFs height ( P = 0.006).
PVR grade C is significantly associated with the incidence and height of ORFs at 1 month after surgery in eyes undergone PPV to repair RRD. Outer retinal folds may provide important insights about the process of retina remodeling occurring in eyes with RRD complicated by PVR C.
