[Clinical comparative analysis of accuracy between tilted and axial implant placement in edentulous jaws assisted by an autonomous dental implant robot].

Abstract

Objective: To preliminarily evaluate and compare the surgical accuracy of tilted and axial implant placement in edentulous patients assisted by an autonomous dental implant robot. Methods: Eight edentulous patients who underwent implant placement assisted by the autonomous dental implant robot at the Affiliated Stomatological Hospital of Nanjing Medical University from November 2023 to November 2024 were enrolled. A total of 42 implants (18 tilted implants and 24 axial implants) were placed. Postoperative cone-beam CT data was obtained and registered with the preoperative planning data. The platform deviation, apical deviation, angular deviation, as well as horizontal and vertical components of the deviations were measured. An unpaired two-tailed t-test was used to compare the accuracy between the tilted and axial groups. Results: The overall mean platform deviation of the 42 implants was (0.67±0.20) mm, the mean apical deviation was (0.73±0.22) mm, and the mean angular deviation was 1.54°±0.54°. The mean values of all directional components were within 0.5 mm. The tilted group showed a significantly greater apical deviation [(0.82±0.17) mm], angular deviation (1.76°±0.43°), and apical horizontal deviation [(0.50±0.13) mm] compared with the axial group [(0.66±0.22) mm, 1.37°±0.55°, (0.42±0.10) mm] (t=2.66, P=0.011; t=2.47, P=0.018; t=2.39, P=0.022). No significant differences were observed in platform deviation, platform horizontal deviation, platform vertical deviation, or apical vertical deviation (all P>0.05). Conclusions: Autonomous dental implant robot-assisted implant placement in edentulous patients achieves satisfactory overall accuracy. Although tilted implants exhibit significantly greater apical and angular deviations than axial implants, all deviation values remain within clinically acceptable ranges.
