Correlation of Systemic Inflammatory Response Index with National Institutes of Health Stroke Scale for Predicting Severity in Stroke.

Abstract

Stroke is a disease associated with high mortality. Systemic Inflammatory Response Index (SIRI) is a descriptive indicator of long-term, low-grade inflammation based on the number of monocytes, neutrophils, and lymphocytes. It has been studied in various diseases such as glioma, breast cancer, and nasopharyngeal carcinoma. However, few studies have been conducted to investigate its role in stroke prognosis, especially in the Indian population. Hence, the present study was done to assess the predictive role of SIRI in stroke patients.
This cross-sectional observational study was conducted at the Department of General Medicine, M. S Ramaiah Medical College and Hospitals, Bengaluru. Patients admitted with a diagnosis of stroke were included. The National Institutes of Health Stroke Scale (NIHSS) score was calculated for these patients. SIRI was calculated by multiplying neutrophil and monocyte counts and dividing the same by lymphocyte counts. The SIRI score obtained was compared with the NIHSS severity score.
A total of 108 patients were included in this study, with most study participants above 60 years of age (61.1%) and a male predominance of 70.4%. The most common comorbidities among patients were hypertension (80.6%) and diabetes (59.3%). The mean NIHSS was 9, and the mean SIRI was 24.9. When Pearson's correlation was done, SIRI was found to be significantly correlated with NIHSS; the correlation coefficient was 0.9365 with 95% confidence interval of 0.9083-0.9563 (P < 0.0001).
SIRI may be a viable and reliable marker for predicting stroke outcomes.
