Abstract
Managing acetabular fractures in the elderly remains a formidable clinical challenge. While traditional approaches include conservative management or internal fixation, the acute total hip arthroplasty or simultaneous "acute fix and replace" strategy-combining fracture fixation with total hip arthroplasty-has gained significant traction and become more prevalent over the past 10-15 years. This narrative review visits the underlying rationale for this integrated philosophy and details various surgical strategies and our preferred technique. Alongside a concise literature review and a discussion of potential complications, we share our preferred surgical tactics, illustrated through case examples.
