Abstract
This study aimed to investigate the efficacy of hyperbaric oxygen (HBO) combined with intratympatic steroid (ITS) treatment for idiopathic sudden sensorineural hearing loss (ISSNHL) in patients with end-stage renal disease (ESRD).
In this retrospective comparative study, 47 ESRD patients with ISSNHL admitted to a municipal tertiary hospital between January 2010 and January 2025 were enrolled. Patients were stratified by treatment into two groups: a combined group (n = 26) treated with HBO plus ITS, and an ITS group (n = 21) who received ITS only. The following outcomes were assessed for intergroup comparison: therapeutic efficacy, hearing recovery, inflammatory factor levels, vertigo and tinnitus improvement, anxiety and depressive symptoms, sleep quality, and adverse reaction incidence. Binary logistic regression was performed to identify factors influencing treatment efficacy in this patient population.
The combined group demonstrated significantly better hearing outcomes, including higher overall response rate and greater improvements in PTA and threshold gains across frequencies, compared to the ITS group (all P < 0.05). Adverse reaction incidence was comparable between groups (P > 0.05). Post-treatment assessments demonstrated the following in the combined group: lower scores on the THI, PSQI, VSS-C, SDS, and SAS scales, along with lower NLR and PLR values. Multivariate analysis identified the treatment delay as an independent risk factor for therapeutic failure.
In summary, early treatment initiation is critical for hearing recovery in ESRD patients with ISSNHL. The combined regimen of HBO and ITS demonstrates superior efficacy, further lowers inflammatory markers, more effectively relieves associated symptoms and improves sleep quality, without increasing adverse reactions.
