Abstract
A 76-year-old woman presented with a 6-month history of progressive anorexia, unintentional weight loss, and heartburn. She reported no abdominal pain, vomiting, gastrointestinal bleeding, or fever but reported intermittent bilateral lower-extremity edema. Physical examination revealed a relatively firm abdominal wall with a palpable upper abdominal mass and no peritoneal signs. Laboratory evaluation demonstrated normocytic anemia (hemoglobin level, 91 g/L; reference range, 115-150 g/L), mild thrombocytosis, no leukocytosis (white blood cell count, 6.24 × 109/L; reference range, 3.5 × 109/L to 9.5 × 109/L), severe hypoalbuminemia (albumin level, 19.5 g/L; reference range, 40.0-55.0 g/L) with low total protein, and electrolyte abnormalities including hyponatremia (sodium level, 128.4 mmol/L; reference range, 137-147 mmol/L) and hypocalcemia (total calcium level, 1.80-1.92 mmol/L; reference range, 2.11-2.52 mmol/L). The d-dimer level was markedly elevated (4900 μg/L [26.8 nmol/L]; reference range, <500 µg/L [<2.7 nmol/L]), whereas serum creatinine level was within the reference range, and serum tumor markers (α-fetoprotein, carcinoembryonic antigen, and carbohydrate antigen 19-9) were not elevated. Contrast-enhanced CT of the abdomen and pelvis (Fig 1) and fluorine 18 fluorodeoxyglucose PET/CT (Fig 2) were performed.
