Centering Community in Evidence-Based Prevention: Enhancing Evidence Criteria and Processes Across a State-Level Prevention System.

Abstract

Evidence-based program registries (EBPRs) are commonly used within state prevention systems to organize and translate prevention science into actionable resources for communities. However, traditional EBPRs often rely on a limited range of methodological standards, which can unintentionally limit the inclusion of programs that may be responsive to community needs and contexts but do not yet meet existing review criteria. This paper describes a multiphase initiative led by the Washington State Health Care Authority (HCA), in partnership with Washington State University researchers, to strengthen statewide evidence review processes for youth substance use prevention programs. First, we outline the evolution of HCA's approach to identifying and supporting evidence-based programs, highlighting efforts to maintain rigor while increasing responsiveness to community needs within state prevention infrastructure. We then describe a four-phase approach that included a focused literature review, consultation with research, policy, and practice experts, development of recommendations, and their operationalization through system-level actions. Key recommendations included establishing clearer pathways for surfacing programs valued by communities for consideration within evidence review processes, advancing a continuum-based approach that recognizes varying levels of evidentiary support, and embedding structured processes for community, practitioner, and system partner engagement and input. Together, this work offers practical guidance for other states seeking to strengthen prevention infrastructure and improve responsiveness to community needs while maintaining rigor through iterative, collaborative approaches.
