Abstract

To provide an evidence-based, practical framework for the detection and management of frailty in community-dwelling older adults in primary care.
Active opportunistic case finding in individuals aged ≥ 70 years using simple performance-based tools enables early identification of frailty. Multicomponent interventions integrating physical exercise, nutritional support, medication optimisation and psychosocial strategies are effective in preventing or reversing frailty.
Systematic frailty management in primary care is feasible and should be based on early detection and personalised, multidimensional care plans.
