Abstract
Obesity in the elderly has become a prominent public health issue. Due to physiological changes and multimorbidity in the elderly population, obesity presents complex clinical manifestations with markedly increased management difficulties. Based on this, National Center for Gerontology, Obesity and Diabetes Group, Chinese Diabetes Society, Chinese Medical Association organized a multidisciplinary panel of experts in obesity and formulated this consensus after multiple rounds of rigorous evaluation and iterative revisions. The consensus emphasizes that the management of geriatric obesity requires a comprehensive assessment tailored to age and overall health status, employing a patient-centered, individualized and stratified management strategy. Following the principle of "safety first, functional improvement, and steady progress", the goal is to reduce visceral fat accumulation, maintain or increase muscle mass, protect bone health, and improve physical function and health status. Special attention should be paid to sarcopenia in the elderly. Adequate intake of high-quality protein foods, combined with a comprehensive resistance exercise program tailored to individual limitations, could effectively improve muscle mass. Furthermore, weight loss medications should be selected based on the evidence of benefit for obesity-related complications or comorbidities, with close monitoring of adverse reactions.
