Developmental Systems Cardiology II: Implementing a Lifespan Model for Clinical Practice.

Abstract

Survival for children with congenital heart disease (CHD) has improved substantially because of advances in surgery, intensive care, imaging, and interventional cardiology. As congenital cardiology evolves into a lifespan discipline, healthcare systems must also address neurodevelopmental vulnerability, psychosocial adaptation, educational disruption, transition complexity, and chronic multisystem disease in adulthood. Although developmental and systems-oriented models of care are emerging, practical guidance for implementation within routine congenital cardiology programs remains limited. This article presents a practical implementation framework for integrating Developmental Systems Cardiology into pediatric and adult congenital cardiology programs through multidisciplinary structures, longitudinal care pathways, neurodevelopmental surveillance, transition medicine, organizational redesign, and measurable quality frameworks. We outline practical approaches spanning fetal diagnosis through adult congenital heart disease care. These include integrated neurodevelopmental follow-up, family-centered communication systems, multidisciplinary Fontan programs, adolescent transition pathways, psychosocial support services, and workforce structures that support longitudinal continuity. Particular emphasis is placed on implementation within real-world pediatric cardiac systems, where resource variation, workforce limitations, and institutional fragmentation frequently challenge coordinated care. Developmental Systems Cardiology moves congenital cardiology beyond isolated episodic models toward integrated longitudinal systems that support adaptation across the lifespan. The framework is intended as a practical and scalable clinical model rather than a prescriptive doctrine. Implementation will appropriately vary according to institutional resources and regional healthcare systems. Future progress in congenital cardiology will depend not only on continued improvements in survival and procedural outcomes, but also on sustainable multidisciplinary systems that support neurodevelopment, family adaptation, educational participation, psychosocial wellbeing, and successful transition into adulthood.
