Abstract

Neurotrophic tyrosine receptor kinase (NTRK) gene fusions are rare but actionable oncogenic drivers in non-small cell lung cancer (NSCLC). Despite the rarity of NTRK fusions, tumor-agnostic tropomyosin receptor kinase (TRK) inhibitors have demonstrated rapid and durable responses across multiple tumor types. In this podcast interview, Justin Gainor and Sanjay Popat discuss the opportunities and challenges associated with developing and implementing targeted therapies for ultra-rare molecular subsets, such as NTRK fusion-positive NSCLC, from a regulatory perspective. Key topics include the evidence leading to the approval of TRK inhibitors, strengths and limitations of basket trials, reliance on single-arm studies and surrogate endpoints, differences between US and UK regulatory and reimbursement frameworks, and the impact of these factors on patient access.
