First Latin American Experience With a Hybrid Dissection Stent in Type A Aortic Dissection.

Abstract

Acute type A aortic dissection (ATAAD) is a life-threatening cardiovascular emergency requiring immediate surgical repair. Hybrid strategies using the Ascyrus Medical Dissection Stent (AMDS) have emerged as adjunctive techniques to improve distal aortic remodeling.
A 63-year-old woman with uncontrolled hypertension presented with sudden chest pain and dyspnea. Computed tomography angiography demonstrated ATAAD extending from the aortic root to the aortic bifurcation, associated with severe aortic regurgitation. The patient underwent hybrid repair with a Bentall procedure combined with AMDS implantation for distal aortic stabilization. Postoperative recovery was uneventful, without neurological complications or reintervention. Follow-up imaging demonstrated false lumen obliteration, preserved visceral perfusion, and favorable aortic remodeling.
This case highlights the feasibility and safety of AMDS as an adjunctive strategy during hybrid ATAAD repair, potentially enhancing distal aortic remodeling without increasing surgical complexity.
AMDS may improve distal aortic remodeling during hybrid repair of ATAAD while maintaining procedural safety and reducing the need for additional interventions.
