Abstract
At some level, intraoperative arterial hypotension causes organ injury. Sicova and colleagues present a meta-analysis of 9 trials in 14 658 patients that compared mortality in patients randomised to routine blood pressure management (with most mean arterial pressures >65 mm Hg) or defined higher pressures. The results were neutral, providing compelling evidence that mean pressures ≥65 mm Hg are generally safe even in older patients with baseline hypertension and other cardiovascular risk factors, and that targeting higher pressures does not reduce organ injury or mortality.
