Mid-term changes detected by corneal topography and optical coherence tomography after prophylactic laser iridotomy in primary angle closure suspect.

Abstract

To evaluate mid term changes in anterior segment parameters and to retinal layers by optical coherence tomography(OCT) in primary angle-closure suspected(PACS) after laser peripheral iridotomy(LPI).
For this retrospective study, 26 eyes of 26 subjects diagnosed with PACS and 20 eyes of 20 age and sex matched healthy people were enrolled. Anterior segment parameters [anterior chamber depth(ACD), anterior chamber volume(ACV), iridocorneal angle(ICA), central corneal thickness(CCT)] measured on Scheimpflug-Placido disc topography system and macular retinal thickness, peripapillary retinal nerve fiber layer(RNFL) thickness and macular ganglion cell-inner plexiform layer (GC-IPL) thickness measured on OCT. All the tests were repeated 1 month and 1 year after LPI.
There was no significant difference between the groups in terms of age and gender. Anterior segment parameters during 1 year ACV, ICA increased, and CCT decreased significantly after LPI(p = 0.038, p = 0.038, and p = 0.020 respectively). A significant thinning of the inner inferior, outer nasal quadrants and the average of all quadrants of the retina was observed (p = 0.001, p = 0.009, and p = 0.001, respectively). The superior GC-IPL thinning was significant(p = 0.037). There was no significant change in the RNFL quadrants at the end of 1 year.
Intraocular pressure can be kept under control with close follow-up in PACS cases and possible progression risk can be reduced by using tools such as laser peripheral iridotomy.
