Measuring What Matters: Defining Compassion in Pediatric Cardiology.

Abstract

Compassion is widely endorsed within healthcare but remains inconsistently defined, variably operationalized, and rarely measured within pediatric cardiology. Contemporary quality systems evaluate mortality, procedural outcomes, complications, and patient experience, but seldom assess relational dimensions of care that may shape family experience and trust. Pediatric cardiology presents a distinctive context: uncertainty may persist across years, high-stakes decisions are frequently made by parents or guardians on behalf of a child, and therapeutic relationships may extend from fetal diagnosis through adulthood. This paper proposes a clinically grounded framework for defining and operationalizing compassion within pediatric cardiology. We define clinical compassion as the intentional recognition of emotional, existential, and clinical vulnerability in children and families, accompanied by relational and behavioral actions aimed at reducing distress, preserving dignity, supporting shared decision-making, and sustaining trust across longitudinal care. Building on existing pediatric compassion and family-centered care literature, we propose six observable domains: recognition of vulnerability, communication under uncertainty, relational presence, shared decision engagement, follow-through and continuity, and humility in action. These domains translate compassion from an abstract value into clinically recognizable behaviors while acknowledging overlap with established dimensions of relational care. We further propose a prototype measurement architecture integrating observed behavioral assessment, child- and parent-reported experience where developmentally appropriate, and clinician reflective practice. The framework is conceptual rather than validated and is intended to support qualitative refinement, multidisciplinary consensus, instrument development, and psychometric evaluation. Compassion should be regarded as an important dimension of clinical practice warranting rigorous definition, evaluation, teaching, and continuous improvement within pediatric cardiology.
