Evaluating the role of radiology in endometriosis in Spain.

Abstract

Endometriosis is a chronic condition affecting approximately 10% of women of reproductive age and is frequently associated with pelvic pain and infertility. Diagnosis can be delayed by up to a decade, partly due to a lack of awareness of the disease among both patients and healthcare professionals. Magnetic resonance imaging (MRI) is a key tool in the diagnosis and management of endometriosis, but accurate interpretation requires experience and the use of appropriate protocols.
To analyse the current state of training, clinical practice and MRI protocols for endometriosis among radiologists in Spain, in order to identify areas for improvement that could help reduce diagnostic delays and optimise management.
A 23-question digital survey was designed, divided into four sections (self-assessment, clinical practice, training and protocol) and distributed via the mailing list of the Spanish Society of Radiology (SERAM).
A descriptive analysis was performed on 160 valid responses. A total of 48.4% of radiologists reported not feeling adequately trained in MRI for endometriosis, and 75% had not received specific training. Over 80% of participants did not have access to multidisciplinary teams at their centre, and 80% did not use structured reporting. Additionally, 24% did not follow a specific MRI protocol for endometriosis, and patient preparation practices varied widely across centres.
Insufficient training, lack of standardised protocols and limited participation in multidisciplinary committees may contribute to delayed diagnosis and suboptimal management of endometriosis in Spain. Addressing these issues could improve the diagnostic and therapeutic approach to this complex condition.
