Abstract
To analyze the relationship between resting heart rate (HR) and body composition indicators and insulin resistance in adults attending primary care.
Cross-sectional study conducted in primary care clinics in Palafolls (Barcelona, Spain). A total of 556 consecutive patients were evaluated; 492 met inclusion criteria and completed all complementary tests. Excluded subjects did not perform the required biochemical or anthropometric assessments. Sampling was consecutive and non-probabilistic, including all patients seen in consultation during the study period who met the inclusion criteria. No interventions were applied. Clinical, anthropometric, and biochemical variables were recorded, including resting HR, body mass index, lean mass, body fat, and HOMA-IR. Resting heart rate, body composition (percentage of fat and lean mass), and insulin resistance estimated by HOMA-IR.
Resting HR ≥75bpm was associated with higher adiposity, lower lean mass, and increased likelihood of insulin resistance (OR≈2,0 across all models). High lean mass showed a protective effect (OR≈0,4). Results remained consistent after adjustment for age and sex.
An elevated resting HR, even within clinically normal values, identifies a less favorable cardiometabolic profile. Resting HR may serve as an independent metabolic marker useful for early detection of cardiometabolic risk in primary care.
