Abstract
Anatomical reconstruction following malunited fractures of the lateral and medial malleolus represents a valuable treatment option with a good long-term prognosis when ankle joint function is still present and should be considered as an alternative to joint replacement or arthrodesis. If revision surgery is not performed ankle osteoarthritis is almost inevitable and joint preservation will eventually no longer be possible. Corrective osteotomy should therefore be performed as early as possible (< 6 months) but is often still beneficial even after several years. Prerequisites for successful revision surgery include an understanding of the pathology, precise imaging including the healthy side and meticulous planning of each surgical step.
