Computed Tomography Angiography Findings and Clinical Characteristics of Aneurysms in Patients with Behçet's Disease: A Retrospective Study.

Abstract

To retrospectively evaluate computed tomography angiography (CTA) findings and associated clinical features of aneurysms in patients with Behçet's disease (BD), with the aim of informing optimal management strategies.
We reviewed BD patients with aneurysms admitted to our institution between 1997 and 2020. All patients underwent pre-treatment CTA. Radiographic features and clinical data were analyzed, with follow-up CTA used to assess outcomes.
Fifty-six patients (median age of 48 years; 82.1% male) were included. BD was diagnosed at a median age of 33 years, with a median interval of 6 years to aneurysm onset. A total of 67 aneurysms were identified: 36 true aneurysms, 29 pseudo-aneurysms, and 2 dissecting aneurysms, most commonly involving the abdominal aorta, popliteal, and iliac arteries. Concurrent thrombosis occurred in 7 patients, affecting lower extremity deep veins, the inferior vena cava, or cerebral venous sinuses. Interventions were performed in 60.7% (34/56), including endovascular stent placement. Among 34 patients with follow-up CTA, 88% showed complete aneurysm resolution.
Vascular complications are frequent and life-threatening in BD. CTA reliably identifies aneurysm type and coexisting thrombosis, supports preoperative planning through anatomical and hemodynamics assessment, and enables accurate monitoring of treatment efficacy and recurrence.
This study provides one of the largest single-center series describing CTA findings in BD-related aneurysms, and highlights the clinical utility of CTA for lesion characterization and longitudinal assessment in this high-risk population.
