Abstract
For patients presenting with pelvic floor defects and pelvic organ prolapse, both conservative measures and surgical pelvic floor reconstruction procedures are available, with the aim of restoring the anatomy and function of the pelvic floor. Conservative therapy (e.g., physiotherapy, pessaries) is frequently employed as a primary approach, whereas surgical interventions are indicated in cases of insufficient effect or severe findings. The choice of treatment is based on a precise diagnostic assessment and classification according to the three compartments of the pelvic floor. Various surgical access routes are available, including vaginal, abdominal or transanal approaches, as well as reconstruction techniques performed with or without mesh augmentation, with an increasing trend toward minimally invasive methods. The objective is to achieve a stable anatomical correction with minimal patient burden and favorable functional outcomes. An interdisciplinary approach is crucial, particularly in cases involving complex, multicompartmental pelvic floor defects. Despite high success rates, complications and recurrences can still occur. Structured follow-up care and pelvic floor training are essential for long-term success.
