Abstract

A 48-year-old healthy man presented with severe inguinal pain and hip immobility after a high-energy injury. Physical examination revealed scrotal invagination and a nonpalpable testis. Imaging confirmed anterior hip subluxation and intra-articular soft-tissue interposition. After failed closed reduction, surgical exploration identified testicular entrapment. Compressive necrosis necessitated orchiectomy; stable joint reduction was achieved. The patient remained asymptomatic at 12 months.
This rare case highlights the necessity of thorough perineal assessment in high-energy pelvic trauma to identify uncommon soft-tissue interpositions preventing joint reduction.
