Right Brain: Language Without Speech.

Abstract

During a neurology rotation as a second-year internal medicine resident, I encountered a preschool-aged child whose absence of spontaneous speech initially framed my clinical thinking. Although she did not answer direct questions, she demonstrated an unexpected facility with written language, reading words from books with ease and engaging with the world through visual and textual cues. Over subsequent encounters, I witnessed the gradual evolution of her communication and the adjustments made by her mother, whose daily life had become shaped by therapies, routines, and the uncertainties surrounding developmental differences. As diagnostic labels accumulated and interventions unfolded, I found myself relying on familiar developmental frameworks and searching for deficits that would explain what I observed. Yet the child's strengths, her self-taught reading, sustained engagement with books, and emerging use of words and phrases challenged those expectations. Progress appeared not through dramatic milestones but through small changes that accumulated over time. The experience reshaped my understanding of communication and clinical observation. Rather than viewing silence solely as absence, I learned to recognize forms of language that existed outside the pathways I had been trained to expect. The encounter also revealed how much of medical training is oriented toward identifying what is missing and how easily strengths can be overlooked when they do not fit conventional frameworks. This reflection explores the distinction between speech and language and the importance of remaining attentive to the many ways meaning can be expressed.
