Effectiveness of video-enhanced educational intervention for fluid management on knowledge, practice, and adherence among patients undergoing hemodialysis: A randomized controlled trial.

Abstract

Fluid management remains a cornerstone of hemodialysis care; however, suboptimal adherence frequently precipitates life-threatening complications, including hypertension and cardiovascular morbidity. While educational interventions are essential to mitigate these risks, empirical evidence regarding the efficacy of video-enhanced instruction in this population remains limited. This study aimed to evaluate the impact of a structured video-enhanced educational intervention on knowledge, practice, and fluid adherence among patients undergoing hemodialysis.
This randomized controlled trial evaluated outcomes using a knowledge questionnaire (reliability 0.88), a practice checklist (0.79), and intradialytic weight gain (IDWG) in 128 hemodialysis patients at South Indian tertiary hospital. Participants (n = 64 per experimental and control group, block randomized) received video-enhanced fluid management education or standard care; analysis included t-test, repeated measures of ANOVA, and Bonferroni tests (P < 0.05 significant).
The experimental group showed significant improvements in mean knowledge scores (11.4 ± 2.2) vs control, P = 0.001) and practice scores (13.6 ± 1.0 vs control 9.1 ± 1.5, P = 0.001) from pretest to post-test 2. Repeated measures ANOVA confirmed differences in practice scores within (F = 102.76, P = 0.001) and between groups (F = 8121.19, P = 0.001), with fluid adherence also improving significantly in the experimental group (P < 0.001).
Video-enhanced instruction is an effective strategy to improve clinical outcomes in hemodialysis patients. Integrating a structured audiovisual tool into routine dialysis care can enhance patient outcomes and support healthcare policy initiatives to improve chronic disease management.
