Extracapsular tunneling for the management of delayed bilateral proximal corporal perforation: Case report and literature review.

Abstract

Inflatable penile prosthesis implantation is generally safe, but delayed bilateral proximal corporal perforation is rare and poorly characterized. We report a case of delayed bilateral proximal corporal perforation after multiple prior IPP revisions, successfully managed with revision surgery using extracapsular tunneling to create a new proximal intracorporal tract for cylinder repositioning. The patient had complete resolution of perineal pain, appropriate cylinder positioning, and satisfactory prosthesis function. This case highlights factors that may contribute to corporal perforation during IPP surgery and demonstrates extracapsular tunneling as a practical reconstructive technique for managing this uncommon complication.
