Abstract

Neovascular age-related macular degeneration (nAMD) is primarily characterized by the formation of abnormal choroidal neovascularization and can lead to substantial visual impairment. Intravitreal administration of anti-vascular endothelial growth factor (anti-VEGF) agents has become the standard first-line treatment for nAMD. However, challenges remain, including the need for frequent injections, considerable treatment burden, and suboptimal responses in a subset of patients. In recent years, the emergence of high-dose anti-VEGF therapies has provided a novel strategy for extending treatment intervals and optimizing long-term disease management. To address these clinical needs, the Ophthalmology Group of China Alliance for Rare Diseases/Beijing Society of Rare Disease Clinical Care and Accessibility organized a multidisciplinary panel of experts to develop this consensus. Based on the latest evidence-based medical findings, together with international guidelines and the realities of clinical practice in China, a standardized consensus-development methodology was employed. Consensus recommendations were formulated for eight key clinical questions regarding the use of high-dose anti-VEGF therapies in the treatment of nAMD. The aim of this document is to provide evidence-based guidance and clinical reference for the standardized application of high-dose anti-VEGF therapies in nAMD, thereby further improving the quality of standardized diagnosis, treatment, and long-term management of nAMD in China.
