Abstract
Low-grade serous ovarian carcinoma is a rare subtype of epithelial ovarian cancer characterized by relative resistance to chemotherapy and limited treatment options at recurrence. Mirvetuximab soravtansine, a folate receptor alpha-targeting antibody-drug conjugate, has demonstrated efficacy in folate receptor alpha-positive ovarian cancer; however, data on low-grade serous ovarian cancer are limited. This study aimed to evaluate the efficacy and safety of mirvetuximab in patients with recurrent low-grade serous ovarian carcinoma.
This is a retrospective review of patients with recurrent low-grade serous ovarian carcinoma treated with mirvetuximab at MD Anderson Cancer Center from 2024 to 2026. Demographic, clinicopathologic, molecular, treatment, and outcome data were collected. Best overall treatment response was assessed using the Response Evaluation Criteria in Solid Tumors, version 1.1, on serial imaging obtained at 3-month intervals. Duration of response and progression-free survival were evaluated.
Twelve patients with recurrent low-grade serous carcinoma were identified. The median age at diagnosis was 47.5 years. Most patients had advanced-stage disease at diagnosis and had received a median of 4 prior lines of systemic treatment. High folate receptor alpha expression (≥75% staining) was observed in 83.3% of tumors. Patients received a median of 13.5 mirvetuximab cycles, and 4 patients received concomitant bevacizumab. The best overall responses were partial response (n = 4, 33.3%), stable disease (n = 7, 58.33%), and progressive disease (n = 1, 8.33%). The disease control rate was 91.67%. The median duration of response among responders was 8.37 months, and the median duration of stable disease was 12.10 months. The median progression-free survival was not reached at the time of analysis because 6 patients remained on treatment. The most common adverse events included ocular toxicity, gastrointestinal symptoms, peripheral neuropathy, and pneumonitis. Toxicities led to dose modifications in 58.33% of cases and drug discontinuation in 25% of patients.
Mirvetuximab demonstrated promising efficacy in patients with recurrent low-grade serous ovarian carcinoma. These findings warrant further prospective studies of folate receptor alpha-targeted antibody-drug conjugates.
