Abstract

Tubo-ovarian abscess (TOA) is a serious complication of pelvic inflammatory disease, most often of polymicrobial origin. It may rarely progress to complicated forms, particularly in cases of delayed diagnosis or in patients with underlying comorbidities. We report a rare complication of TOA characterized by subcutaneous abdominal wall fistulization, associated with an unusual Klebsiella pneumoniae infection. This pathogen, classically involved in urinary and gastrointestinal infections, is rarely described in TOA. Management combining drainage of the collection and prolonged targeted antibiotic therapy resulted in a favorable outcome.
