Abstract

Renal-limited thrombotic microangiopathy (TMA) is clinically milder but etiologically complex and is often underdiagnosed. This case presents as complement-mediated TMA with malignant hypertension and monoclonal gammopathy but showed suboptimal response to eculizumab treatment, suggesting heterogeneity in both etiology and treatment outcomes.
