Abstract
This retrospective cohort investigated factors associated with non-adherence to antiretroviral therapy (ART) among pregnant women living with human immunodeficiency virus (HIV) using ART in Brazil between 2014 and 2019. Adherence was defined as <80%. The databases were: the Drug Logistics Control System, the Laboratory Test Control System for Lymphocyte Count and HIV Viral Load, and Data and Knowledge Integration Center database. Associations were evaluated using univariate logistic regression, and variables with p < 0.20 were included in the multivariate analysis. The study included 23,757 pregnant. Non-adherence was 20%, associated with primigravidas (OR = 1.13, p = 0013), young women (OR = 1.29, p = 0 000), non-white women (OR = 1.24, p = 0000), single women (OR = 1.18, p = 0000), women with low educational attainment (OR = 1.27, p = 0000), women with changes in ART regimen (OR = 1.34, p = 0000) or adverse reactions (OR = 1.29, p = 0004), and areas with very high deprivation (OR = 1.22, p = 0006). The results highlight sociodemographic and socioeconomic vulnerabilities among pregnant living with HIV in Brazil, emphasizing the need for intersectoral public policies to reduce inequalities and strengthen adherence to ART among the most vulnerable.
