Abstract

to evaluate the factors influencing the evolution of health status of dental implants with a laser micro-grooved collar surface (LaserLokÒ) during 10 years of implant function, in patients enrolled in preventive maintenance and attending a private practice.
Dental records of 60 patients receiving 240 implants (112 tissue level, 128 bone level) with LMS were retrospectively evaluated. Patient-, implant/prosthetic- and maintenance-related factors influencing changes of peri-implant tissue conditions were analyzed using a generalized estimating equation (GEE).
Patient health conditions remained the same in 42% of cases (35% with peri-implant health (PH) and 7% with peri-implantitis (P), worsened in 36% (29% from PH to peri-implant mucositis (PIM), and 7% from PH to P) and improved in 22% (15% from PIM to PH and 7% from PH to peri-implant marginal bone gain). PIM tended to increase in subjects with history of periodontitis, difficulty cleaning around the implant, and higher plaque scores, while P tended to increase in patients who were smokers or were affected by systemic diseases. Moreover, the worsening in PIM was also associated with prosthetic emergence angle 30°, while the worsening in P was also associated with cemented crowns.
Peri- implant disease progression over time is influenced by modifiable patient-, prosthetic-, and maintenance-related factors, highlighting the need for personalized care and proactive risk factor management to prevent disease progression.
