Abstract
To explore the surgical treatment and effectiveness of tibial plateau fracture with intra-articular tiny free bone fragments.
A clinical data of 45 cases of tibial plateau fracture with intra-articular tiny free bone fragments (<5 mm) between October 2023 and November 2024 was retrospectively analyzed. All patients underwent open reduction and internal fixation. The tiny free bone fragments were removed in 21 cases (removal group) and re-implanted in 24 cases (re-implantation group). There was no significant difference in baseline data, such as gender, age, cause of injury, Schatzker classification of fractures, and time from fracture to surgery between groups ( P>0.05). The surgical time, intraoperative blood loss, hospital stay, fracture reduction quality, fracture healing time, visual analogue scale (VAS) score for pain, Hospital for Special Surgery (HSS) score for knee function, and incidence of postoperative complications were recorded and compared between groups.
There was no significant difference between groups ( P>0.05) in the intraoperative blood loss or hospital stay, but the surgical time was significantly shorter in the removal group than in the re-implantation group ( P<0.05). All patients were followed up, and the follow-up time was (15.24±2.07) months in the removal group and (15.08±2.02) months in the re-implantation group, with no significant difference ( t=0.253, P=0.801). There was no significant difference between groups ( P>0.05) in fracture reduction quality, fracture healing time, HSS score for knee function and VAS score for pain at last follow-up. During follow-up, superficial infection of incision occurred in 1 case in the removal group and 2 cases in the re-implantation group, and delayed union of fracture occurred in 1 case in each group. There was no significant difference in the incidence of complications between groups ( P>0.05).
For the tibial plateau fracture with intra-articular free bone fragments, the prognosis does not be affected after removal of tiny free bone fragments (<5 mm) during surgery.
