Abstract

Respiratory syncytial virus (RSV) affects all ages, with infants and older adults most affected by severe disease. The RSV prevention landscape includes 39 candidates in clinical development, alongside setbacks such as the discontinuation of a promising paediatric live-attenuated vaccine candidate. Several products have been approved, including a maternal vaccine, infant monoclonal antibodies, and vaccines for older adults and adults at high risk. Due to high cost, implementation of these products remains limited to higher-income settings. Real-world evidence confirms high effectiveness, with emerging benefits beyond respiratory disease, including reduced cardiovascular events in older adults. Despite comparable effectiveness of maternal vaccination and monoclonal antibodies, differences in uptake highlight that product availability alone will not ensure population impact. Achieving uptake requires integration into accessible routine care, recommendation by trusted providers, and context-sensitive delivery strategies. This Review summarises RSV preventive products, real-world effectiveness and uptake, and the opportunity of using communication in translating RSV prevention into impact.
