Abstract
Cutaneous malignant melanoma is an aggressive malignancy with a high metastatic potential, and although ovarian metastasis is extremely rare, it may be easily overlooked or misdiagnosed due to its nonspecific clinical manifestations. Here, we report a rare case of bilateral ovarian metastases from cutaneous malignant melanoma in a 51-year-old female patient who presented with abdominal pain one year after surgical resection of the primary skin lesion. Pelvic imaging revealed bilateral ovarian masses measuring 12 × 10 cm on the left side and 10 × 9 cm on the right side. The patient subsequently underwent total hysterectomy, bilateral adnexectomy, and resection of the greater omentum. Histopathological examination combined with immunohistochemical staining confirmed metastatic malignant melanoma of the ovaries, with tumor cells showing positive expression of SOX10, HMB45, and Melan-A. Molecular analysis revealed a BRAF mutation, and subsequent magnetic resonance imaging demonstrated multiple brain metastases. The patient was treated with whole-brain radiotherapy combined with vemurafenib-targeted therapy.
