Abstract

A 26-year-old woman presented with palpitations and exertional dyspnea. Echocardiography suggested an inferior sinus venosus atrial septal defect, while cardiac CT angiography revealed a 26 × 32 mm defect with inferior vena cava overriding, a persistent left superior vena cava draining into the left atrium, and a coronary sinus with an intact roof draining independently into the left atrium. Surgical correction involved patch closure of the atrial septal defect and redirection of both venous structures to the right atrium. Postoperative imaging demonstrated no residual interatrial shunt and confirmed patency of the redirected persistent left superior vena cava. This case highlights the essential role of multimodality imaging in defining complex congenital venous anatomy and guiding surgical management .
