Abstract
Sorafenib is an oral multikinase inhibitor approved for advanced hepatocellular carcinoma, renal cell carcinoma, and maintenance therapy in selected patients with FMS-like tyrosine kinase (FLT3)-mutated acute myeloid leukemia following allogeneic hematopoietic stem cell transplantation. We report the case of a 35-year-old woman with FLT3 internal tandem duplication (FLT3-ITD) and nucleophosmin 1 (NPM1)-positive acute myeloid leukemia (AML) who developed painful hyperkeratotic plantar plaques approximately eight weeks after initiating sorafenib therapy. Clinical examination revealed multiple symmetrical hyperkeratotic plaques involving the pressure-bearing areas of both soles, associated with pain and tenderness, resulting in significant impairment of daily activities. She was diagnosed with sorafenib-induced hand-foot skin reaction (HFSR) and was managed with topical keratolytic and corticosteroid therapy. This case highlights the importance of early recognition and prompt management of sorafenib-induced HFSRs to minimize morbidity and maintain oncological treatment.
