Abstract

Hereditary diffuse gastric cancer syndrome, recently renamed diffuse gastric and lobular breast cancer (DGLBC) syndrome, is an autosomal dominant condition associated with increased risk for DGLBC. As our understanding of this condition has evolved, so have recommendations for diagnosis and management. Widespread use of multigene panel testing and recognition of variable penetrance between families with CDH1 pathogenic variants have led to expansion of eligibility for genetic testing. Simultaneously, lower gastric cancer penetrance estimates and better understanding of the risks of prophylactic gastrectomy have led to increased acceptance of endoscopic surveillance as an alternative for many patients.
