Abstract

To construct a standardized whole-process management model for liver cancer patients undergoing Yttrium-90 selective internal radiation therapy (Y-90 SIRT) and evaluate its clinical efficacy, safety and impact on medical resource utilization.
This retrospective cohort study included patients with primary or secondary liver malignancies who received Y-90 SIRT at the First Affiliated Hospital of Jinan University between August 2022 and October 2025. Patients were assigned to a control or an intervention group according to the timing of model implementation. Adverse events (AEs), psychological status, satisfaction, health-related quality of life (HRQoL), length of hospital stay (LOS) and hospitalization costs were compared between groups.
In total, 307 patient episodes were included (144 control, 163 intervention), with balanced baseline characteristics (all P > 0.05). The overall incidence of AEs was significantly lower in the intervention group than in the control group (14.11% vs 40.27%, P < 0.001), as was that of common AEs (11.65% vs 34.72%, P < 0.001); specific AEs did not differ. Compared with the control group, the intervention group had significantly lower postoperative SAS (42.7±6.6 vs 48.2±9.0; P < 0.001) and SDS scores (48.6±6.6 vs 50.9±8.1; P = 0.001), higher satisfaction (25.63±2.82 vs 23.10±2.95; P < 0.001) and better emotional well-being (9.00 [8.00, 11.00] vs 8.00 [7.00, 10.00]; P = 0.015). Length of hospital stay did not differ, whereas hospitalization costs were lower in the intervention group (P = 0.013).
In this single-center retrospective cohort, the whole-process management model was associated with fewer adverse events and with better patient satisfaction and emotional well-being in patients undergoing Y-90 SIRT. Because long-term oncological outcomes were not assessed, these findings are hypothesis-generating and require prospective confirmation.
