Abstract

Over 80% of cases of clinically advanced ovarian carcinomas (CAOC) contain inactivating genomic alterations in the TP53 tumor suppressor gene. Among these, the rare TP53 Y220C hotspot mutation can be potentially "reverted" to restore wild-type function using novel drugs, such as Rezatapopt (PC14586, Clinical Trial NCT04585750). This study builds on existing knowledge by exploring the molecular changes associated with TP53 Y220C-positive CAOC. A retrospective review of specimens from the Foundation Medicine database identified 29,365 cases of CAOC of various subtypes that had undergone hybrid capture-based comprehensive genomic profiling. Within the 29,365 cases, 854 (2.9%) featured a TP53 Y220C mutation (Y220C+). When compared with the 28,511 CAOC negative for TP53 Y220C mutations (Y220C-), the Y220C+ patients had similar increased frequencies of genomic alterations in CCNE1, KRAS, PIK3CA, NF1, and BRCA1. CAOC features a 2.9% incidence of the potentially targetable TP53 Y220C mutation, making it one of the most frequent tumor types harboring this rare but recurrent mutation.
