Abstract

Follicular lymphoma (FL) is a prototypical indolent B-cell non-Hodgkin lymphoma characterized by prolonged survival but a relapsing, largely incurable clinical course. Advances in molecular biology have elucidated key mechanisms of disease pathogenesis, including t (14;18) translocation, recurrent epigenetic alterations, the tumor microenvironment, and clonal evolution. In clinical practice, frontline treatment strategies based on disease stage and tumor burden have been increasingly refined to greater emphasize safety and tolerability. Notably, the emergence of novel immunotherapies, particularly chimeric antigen receptor T-cell (CAR-T) therapy and bispecific antibodies, has brought about a paradigm shift in the management of relapsed or refractory FL. In addition, a novel anti-CD19 monoclonal antibody has further expanded therapeutic options. Integration of these immunotherapies into earlier lines of treatment and development of combination strategies are expected to further improve outcomes. Future challenges include defining the optimal positioning and sequence of these therapies and advancing personalized treatment approaches to maximize long-term disease control.
