Abstract
Primary hyperparathyroidism (PHPT) is a common clinical endocrine and metabolic disorder. Its clinical phenotype spectrum is undergoing a profound shift, evolving from the classic, severe presentations characterized by bone and kidney involvement towards more occult and subclinical forms. With the increasing adoption of routine biochemical screening and advancements in diagnostic technologies, there has been a notable rise in subclinical phenotypes of PHPT in China, particularly asymptomatic PHPT and normocalcemic PHPT. This trend not only reflects the progress of Chinese public health system but also imposes higher demands for precision in clinical diagnosis and treatment. Meanwhile, preoperative localization techniques have evolved from traditional ultrasound and radionuclide scanning to the era of four-dimensional CT and choline positron emission tomography, laying a solid foundation for the widespread adoption of minimally invasive parathyroidectomy. However, the clinical management of PHPT in China still faces significant challenges, including insufficient awareness among primary care physicians, imbalanced accessibility to advanced imaging equipment, and the complexity of managing multi-gland disease. This review systematically examines relevant literature published both domestically and internationally in recent years. It aims to outline the theoretical evolution, technological advancements, and disease characteristics specific to the Chinese population, with the goal of providing a reference for the standardized diagnosis and treatment of PHPT in China.
