Abstract
Medication use largely occurs outside healthcare: 98% of defined daily doses of medicines in Sweden are managed by individuals at home. Patients themselves therefore carry a major responsibility for safe and effective treatment, while the role of the health care team is to support the patient. Proper use depends on clear, individualised information about what medicines to use and how to take them, as well as possible side-effects and treatment goals. Patient-centered communication improves adherence, understanding, and outcomes. Still, non-adherence remains common and multifactorial, involving both intentional and unintentional behaviours. Adherence is governed by patient factors as well as healthcare system and societal factors. Medication use at home also entails risks, such as improper storage, dosing errors and poisoning. At the same time, patient empowerment and self-management of drug treatment, including home monitoring of treatment effects, has the potential to improve outcomes and reduce healthcare burden.
