Abstract

Opioids are widely used in clinical practice for the management of moderate-to-severe acute and chronic pain. However, their use is associated with the development of tolerance and physical dependence, often followed by a withdrawal syndrome upon discontinuation. The mechanisms underlying opioid dependence and withdrawal are highly complex and remain incompletely understood. These processes involve multiple regions of the central nervous system and are accompanied by widespread alterations in neuronal signaling pathways. Neurotransmitters, which are essential for communication between neurons, play a critical role in opioid addiction and withdrawal. In this review, we provide a comprehensive overview of the major neurotransmitter systems, their involvement, and the changes that occur during acute and chronic opioid exposure. Particular attention is also given to the consequences of opioid discontinuation. Overall, this review summarizes current knowledge of the molecular mechanisms underlying both the acute and long-term effects of opioids on key neurotransmitter systems in the brain.
