Abstract

This study aimed to investigate the species distribution, clinical manifestations, antimicrobial susceptibility, and mortality-related inflammatory risk factors of Aeromonas infections in the Zhoushan Archipelago, China.
A retrospective cohort study was conducted on Aeromonas strains isolated from clinical specimens between 2019 and 2024. Species were identified using MALDI-TOF MS, and antimicrobial susceptibility was tested via VITEK 2 Compact.
Among 173 cases, A. sobria (31.21%) and A. hydrophila (31.79%) were predominant. A. sobria was associated with a significantly higher rate of poor prognosis (9.26% vs 1.68%). All 7 fatal cases developed sepsis and displayed severe systemic inflammation, evidenced by sharply decreased red blood cells, platelets, and albumin, as well as elevated creatinine (all P < 0.01). A. hydrophila showed higher resistance to cefazolin and cefoxitin, while amikacin and tobramycin remained highly susceptible.
In the Zhoushan Archipelago, A. sobria poses a pronounced lethal risk, particularly in immunocompromised patients, through rapid progression to septic shock driven by unchecked inflammatory responses. Ceftazidime combined with amikacin is recommended as empirical therapy. The study highlights the need to monitor early inflammatory markers for improving outcomes.
